Positive Behavioral Support

Positive Behavioral Support (PBS) is a holistic approach that aims to improve quality of life while reducing undesirable behaviors. This is achieved by replacing negative behaviors with newly acquired skills and proactively eliminating triggering events from the individual's environment. PBS is an evolving methodology that draws inspiration from applied behavior analysis, person-centered values, and the normalization/inclusion movement. Its ultimate goal is to create a lifestyle that promotes positive behavior and enhances overall well-being.

Our aim is to assist individuals in achieving the life they desire by fostering a therapeutic environment that incorporates person-centered practices and positive behavior supports. We strongly encourage active engagement from both the individual and their team in developing a personalized behavior plan. Furthermore, we firmly believe that a person-centered plan serves as the foundation of the process and informs the behavior support plan.

What Services are Offered?

For each applicant, our team ensures a thorough and comprehensive assessment of their functional abilities, conducted by a licensed professional. Following the assessment, we proceed to develop and implement a positive support plan tailored to the individual's specific needs. This plan incorporates proactive strategies aimed at promoting positive actions and behaviors.

Our service offers three levels of staffing, including the Professional, Analyst, and Specialist, with varying hours based on the individual's needs. We understand the importance of flexibility, and our staff can provide support within the desired hours and timeframes of the individual.

Our experienced team has identified several areas where individuals may benefit from our support, including:

  1. Companionship in the community: We foster social connections and support individuals in engaging with their communities, promoting a sense of belonging and well-being.

  2. Skill building: We focus on developing new skills and enhancing existing ones to empower individuals to achieve their goals and increase their independence.

  3. Self-advocacy and self-sufficiency: We provide guidance and training to enable individuals to advocate for themselves and become more self-sufficient in managing their daily lives.

  4. Coordinating medical and mental health resources and care: We assist individuals in accessing the necessary medical and mental health services, ensuring they receive comprehensive support.

  5. Acquiring skills for self-management of disability-related supports: We work with individuals to develop skills that allow them to take control and responsibility for managing the supports they receive, such as Home and Community-Based Services (HSS) and Personal Care Assistance (PCA).

  6. Identifying community supports and action items for long-term independence: We collaborate with individuals, like Johnny, to identify community resources and create action plans to achieve their long-term vision of independence, with a focus on reducing hospital visits and injuries.

At Affinity Care, our dedicated team is committed to providing personalized and comprehensive support that empowers individuals to reach their full potential and lead fulfilling lives.
