The Role: Program Coordinator - Affinity Care is looking for a designated coordinator to join our supportive housing team. The ideal candidate will be familiar with 245D standards and are passionate about providing person-centered support and empowering individuals with mental illness, intellectual and developmental disabilities.

Location: Possible travel between Bloomington, St. Paul, and Minneapolis​

Pay: This is a salaried role - $45,000.00 - $55,000.00 per year

Hours: Full Time - 40 hours per week
Essential Duties and Responsibilities of Affinity Care's Program Coordinator:

Participating in the development, refinement, and implementation of individualized program plans that will help individuals safely achieve their goals.

Providing work direction, guidance, training, and feedback to staff to ensure the highest standards of service delivery are provided to the individuals.

May need to assist with providing direct care to individuals such as bathing, toileting, hygiene, medication/treatment administration, providing escorts and mobile supports and behavior interventions.

Contributing to the safety and security of the site, individuals served, and co-workers.
What Kind Of Qualifications Do You Need?
Must meet ONE of the qualifications to be a Program/Designated Coordinator:

Bachelor’s degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR

Associate’s degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
-
A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
-
A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older under the supervision of a staff person who meets the qualifications in one of the categories above.
Available for on-call and weekend shift support.
Must be willing to submit to a background study completed by the Department of Human Services.
Must have a valid driver's license, insurance, and a good driving record.
