Affinity Care Program Coordinator

  • The Company: Affinity Care MN is a fast-growing Assisted Living & Home Care company helping individuals with disabilities live with dignity with our exceptional care.​

  • The Role: Program Coordinator - Affinity Care is looking for a designated coordinator to join our supportive housing team. The ideal candidate will be familiar with 245D standards and are passionate about providing person-centered support and empowering individuals with mental illness, intellectual and developmental disabilities.

  • Location: Possible travel between Bloomington, St. Paul, and Minneapolis​

  • Pay: This is a salaried role - $45,000.00 - $55,000.00 per year

  • Hours: Full Time - 40 hours per week

Essential Duties and Responsibilities of Affinity Care's Program Coordinator:

​

  • Participating in the development, refinement, and implementation of individualized program plans that will help individuals safely achieve their goals.

  • Providing work direction, guidance, training, and feedback to staff to ensure the highest standards of service delivery are provided to the individuals.

  • May need to assist with providing direct care to individuals such as bathing, toileting, hygiene, medication/treatment administration, providing escorts and mobile supports and behavior interventions.

  • Contributing to the safety and security of the site, individuals served, and co-workers.

What Kind Of Qualifications Do You Need?

Must meet ONE of the qualifications to be a Program/Designated Coordinator:

  • Bachelor’s degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR

  • Associate’s degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR

  • A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR

  • A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older under the supervision of a staff person who meets the qualifications in one of the categories above.

Available for on-call and weekend shift support.

Must be willing to submit to a background study completed by the Department of Human Services.

Must have a valid driver's license, insurance, and a good driving record.

Must be willing to submit to a background study completed by the Department of Human Services.

Must be willing to travel to different site locations.
