Information that we collect

Affinity Care MN does not lease or sell the personal information you provide to us to any third party.


Affinity Care MN does not automatically collect information that would personally identify you unless you decide to provide it. By way of example and without limitation, if you choose to use one of our online, automated forms on the Site, except as required by law, the information that you provide will be shared only with those people within Affinity Care MN who need this information to respond or to otherwise decide what appropriate action, if any, is to be taken with regard to your communication, those persons within Affinity Care MN that will utilize the information to maintain and improve Affinity Care MN’ operations, and with third-party service providers or suppliers whom we have engaged. In circumstances where we have engaged service providers or suppliers, we will first obtain the agreement from those third parties not to use or disclose your personal information to the extent required by applicable law, except to help us provide services.


Use of personal information by third parties whom we engage may include, by way of example and without limitation, reviewing the information collected through online automated forms to repair or maintain technical functionality or take action with regard to any inappropriate use of the Site.

IP Addresses

Each internet protocol address (IP address) of each computer that accesses the Site is automatically collected by the Web server. The IP address is a unique number that is assigned to designate a computer every time the computer is used to access the Internet. Because many different persons can access the Internet via the same computer, and because it is possible to obtain a different IP address for a computer, the IP address does not definitively, conclusively identify the person using the Site.
