Affinity Care MN provides on-call counseling, problem-solving, and rapid response for health or personal emergencies that are not related to an injury or immediate danger. This service does not replace 911—please call 911 for any life-threatening emergency.
What We Offer
Specialist services are tailored to meet the unique needs of individuals based on their specific circumstances and challenges. The interventions are designed to be holistic and comprehensive, covering a wide range of support areas that empower individuals to live more independently and engage more fully in their communities. Below are some examples of how specialist services can be applied in various areas:

Immediate response to assist you at home (in person or through remote/phone support, based on your support plan).

On-call counseling to talk through concerns and de-escalate situations.

On-call problem-solving to help you decide next steps and connect the right supports.

Personal Emergency Response System (PERS): installation, device, monitoring, and testing when equipment support is authorized.
Who is Eligible
You may qualify for 24-Hour Emergency Assistance if either of the following applies:
You live in your own home, do not receive 24-hour supervision, and would otherwise require extensive, routine supervision without this service.
You live with a primary caregiver who would otherwise need extensive support to secure help during an emergency.
What’s Not Included
This service does not cover:​
Overnight staffing
(awake or asleep)
On-call support that is scheduled or part of other services
Staffing for other programs
You cannot receive this service along with 24-hour customized living, community residential services, family residential services, or integrated community supports.
We are a DHS-enrolled provider for non-equipment 24-hour emergency assistance and meet all applicable Minnesota licensing requirements.
