Community Residential Setting

At Affinity Care MN, our Community Residential Services (CRS) provide licensed, structured residential care for adults who benefit from ongoing support in a safe and nurturing environment.

CRS offers individualized training, habilitation, and supportive services designed to help each person develop skills, maintain stability, and live as independently as possible. All services are based on the individual’s support plan and tailored to their unique strengths, needs, and goals.

What is CRS?

A Community Residential Setting is a licensed home that serves adults in a structured, team-supported environment. In this setting, the license holder does not live in the home. Instead, trained staff work scheduled shifts to provide 24-hour care and supervision.

At least one resident in the home receives services funded through a Home and Community-Based Services (HCBS) waiver program.

Within our CRS homes, individuals receive:

  • Individualized residential care based on their support plan

  • Skill-building and habilitation services

  • Medication management and health support

  • Assistance with daily living activities

  • Community engagement and integration support

What Services are Offered?

Our organization specializes in offering round-the-clock, community-based Customized Living housing and services specifically tailored for adults facing the challenges of Serious and Persistent Mental Illness (SPMI) or other disabilities. We firmly believe in enabling individuals to uphold their independence while actively participating in the larger community. Our conveniently located facilities can be found in both Minneapolis, Bloomington, and Richfield.
