Integrated Community Supports
At Affinity Care MN, we recognize that housing needs change across different phases of life—and every person deserves a space where they can feel safe, supported, and seen. That’s why we offer a range of supportive housing options designed to meet people where they are. Whether someone is ready for more independence or needs round-the-clock care, we provide housing models that honor their goals and help them thrive.

From Integrated Community Supports (ICS) that promote independent living with flexible support, to Community Residential Services (CRS)—including customized and assisted living settings—for individuals who benefit from structured, 24/7 care, our homes are more than places to live. They’re places to grow, heal, and belong.
What is ICS?
Integrated Community Supports (ICS) is a service designed for adults with disabilities who live in apartment-style housing controlled by a provider. This model prioritizes privacy, autonomy, and flexible support based on the person’s goals and needs—allowing individuals to live more independently while still receiving customized assistance.
How We Support
-
We help develop and implement individualized support plans.
-
Our staff provide on-site assistance with daily living skills, employment goals, health needs, and community engagement.
-
We encourage independence while remaining accessible for meaningful, hands-on support.
ICS is ideal for individuals who are ready to live on their own or with minimal co-tenancy but still need consistent access to supports that promote stability, connection, and personal growth.
