Targeted Case Management

At Affinity Care, our Targeted Case Management (TCM) program is designed to support adults living with mental health challenges in navigating the many systems that affect daily life. TCM is a person-centered care coordination service that helps you identify needs, set meaningful goals, and access the resources required to achieve stability, independence, and improved quality of life.

What is Targeted Case Management?

Targeted Case Management is a county-authorized mental health service designed to support adults who need assistance coordinating care and accessing community resources.

At Affinity Care, our case managers walk alongside you to:

  • Identify your strengths and needs

  • Develop a clear, goal-driven plan

  • Connect you with essential services

  • Coordinate providers and supports

  • Monitor progress and adjust as life changes

Our approach is relationship-based, culturally responsive, and focused on preserving dignity while promoting long-term stability and recovery.

Who We Serve

Our TCM services are available to adults who:

  • Are diagnosed with a serious and persistent mental illness

  • Are eligible for Medical Assistance (MA)

  • Need support accessing and coordinating services

  • ​Experience barriers in housing, employment, healthcare, or daily functioning

To receive Targeted Case Management through Affinity Care, contact your county and request a mental health case manager.
