Waiver Case Management

Navigating waivers, services, and providers can feel like a lot to hold on your own. You don't have to. At Affinity Care, your case manager is one steady person who helps you understand your options, plan with purpose, and stay connected to the support that makes independent living possible.

What is Waiver Case Management?

Waiver case management is a service for people enrolled in a Minnesota disability or home- and community-based waiver. Your case manager is the throughline across every part of your support, the person who keeps your plan moving and makes sure it still fits your life.

At Affinity Care, your case manager walks alongside you to:

  • Identify your goals, strengths, and needs

  • Develop a clear, person-centered support plan

  • Connect you with the right services and resources

  • Coordinate the providers and supports around you

  • Monitor progress and adjust as life changes

Waiver Programs We Support

If you're enrolled in one of these programs through Ramsey County, we can serve as your case management provider.​

Brain Injury (BI)

Support for people living with the lasting effects of a traumatic or acquired brain injury.

Community Access for Disability Inclusion (CADI)

Services that help adults with disabilities stay active and independent in their community.

Developmental Disabilities (DD)

Lifelong, person-centered planning for people with intellectual and developmental disabilities.

Elderly Waiver (EW)

Community-based supports for older adults who choose to stay in their own homes.

Consumer-Directed Community Supports (CDCS)

CDCS is a flexible option within a waiver that gives you more say over how your budget and supports are used. If you're not sure which program fits, an MnCHOICES assessment will determine your eligibility, and we're glad to help you understand the process.

How to Get Waiver Case Management Services

To receive waiver case management through Affinity Care, you'll first need a MnCHOICES assessment to confirm eligibility, then you can request us as your provider.

Request a MnCHOICES assessment. Contact Ramsey County's disability services intake to ask whether you qualify for a waiver program and arrange your assessment.

Ask for Affinity Care. During your assessment, let your assessor or county worker know you'd like Affinity Care as your waiver case management provider.

We get started. Once you're approved and assigned to us, we'll connect you with your case manager and begin building your plan.
